NASN needs your help with data collection for EveryStudentCounts!
For this collection cycle, we are asking nurses/districts to submit data from the 2025–2026 school year using three Microsoft Forms:

School-Level Health Office Visit Dispositions

Chronic Health Conditions of Students
We are not collecting any student-specific information. No student names, birthdates, ID numbers, individual health records, or other personally identifiable student information should be submitted. The forms only ask for school-level or aggregate counts from the 2025–2026 school year, so student privacy should not be a barrier to participation.
