Funeral Director's Association (AZFDA)
Funds School Nurses to Assist Administering Funds updated: (10/10/07) The Arizona Funeral Directors Association Foundation For Children was established in 1998 to help children in our state. When there is no other available resource, school nurses are able to contact the Arizona Funeral Directors Association - Foundation for Children and access funds to make a specific purchase. "With the expenditure of a relatively small amount of money, the health and school attendance of children can be improved. The AFDA Foundation is there to provide funding for such needs." -- Ron Adair 623-694-8558 Fax: 623-979-2775 E-mail: sklrnmary@aol.com SNOA's goal is to make information about this valuable resource to ALL nurses in the state. Please pass the word to your school nurse colleagues. SNOA will have Forms available at the monthly Net "working" meetings (see calendar page). A few things to note when making requests: 1. Only medical treatment, health care or dental needs for under privledged children ages 5 - 17 will be considered. Mental Health requests are not considered. 2. Requests for funding must be made prior to the actual visit, medical test, examination or medical treatment. Reimbursements after the fact are not generally made, but may be considered by the Board in emergency situations. 3. Specific requests to aid eyesight or hearing may be referred. Hearing is referred to the EAR Foundation. Vision requests with a social security # are referred to Prevent Blindness. Without a social security #, vision requests are referred to Nationwide. At Nationwide, parents will be responsible for 10% of the cost and two pair of glasses will be provided. 4. Emergency medications (one time request only) may be granted; ongoing medications are not funded. Ongoing medical care is not provided. 5. Requests for clothing, bedding ir household items are not funded. Requests for burial if a deceased child are not considered; however, most funeral homes will consider reduction in price or donations of some services. It is best to check with several funeral homes for this type of request. 6. Requests for more than $250 will go to the Board of Directors for review before approval. Please submit detailed information regarding the need as possible so the Board can make an informed decison on the request. The Board meets on the 2nd Thursday of each month. 7. Requests are only taken between the months of August 1 to May 1. Remember this fund is not Phoenix, Tucson, Flagstaff or Yuma exclusive. AZFDA would like to reach every school in the state of Arizona. Your local funeral home has assisted in providing the funds for the Foundation through personal contributions and community events. We invite you to contact them and develop a relationship. Funeral service professionals are caring, committed individuals who support your efforts in helping the welfare of Arizona's children.
